EVERGREEN SCHOOL DISTRICT STUDENT REGISTRATION

Are you new to the Evergreen School District? Please follow the steps below to enroll your child(ren).

STEP #1 - Is your residence within the Evergreen School District? Not sure, you can check your address here: "Address Checker"

  • No, please fill out the "Out-of-District Student Enrollment Request Form" (You will need to complete one form per child)

  • Yes, proceed to Step #2

STEP #2 - Please gather the following items before proceeding to Step #3

  • Household Information - a digital copy of proof of residency (utility bill, rental/lease or mortgage agreement), address, and phone numbers

    • Living with another person, please upload these two forms as your proof of residency. Affidavit of Residence Form (English)/(Spanish) and OPI Student Residency Form (English)/(Spanish)

  • Parent Information - home, work, cellphone numbers and email addresses

  • Student Information - demographic/digital copy of the state-certified birth certificate, a digital copy of immunization records, and health/medication information Affidavit of Exemption on Religious Grounds (English)/(Spanish)

  • Emergency Contacts - addresses and phone numbers of emergency contact and pick-up persons

  • Previous School Information - (School name, Address, Phone #, and Fax #)

  • Secondary Household Information - for a parent that resides in a secondary household

  • Court Documents (if applicable) - Email any court documents (Parenting Plan, Order of Protection, guardianship paperwork, or case worker information) to registrar@evergreensd50.com.

STEP #3 - Select the Link Below to Start an Application (PRESCHOOL and EARLY LITERACY enrollment must be pre-approved through our Special Services Department. Please call 406-751-1111 for more information. CROSSROADS enrollment must be pre-approved.  If not pre-approved, your application will be denied.)
